Nadal suffered a surprise 6-3 7-6 (8-6) defeat at the hands of Feliciano Lopez in the second round at the ATP 1000 event on Wednesday, but that will not prompt him to go under the knife before the end-of-season finals in London.

The Spaniard has already confirmed his place in the November tournament and is confident a course of strong antibiotics can help him keep the problem under control and avoid surgery.

Lopez out-ran and out-played the world number one in China and after the defeat Nadal, who was the top seed, admitted he had struggled to compete because of the problems he has had to contend with.

"For sure [I don't feel] perfect," said Nadal. "It's the same way that I felt before.

"A little bit more dizzy now. But that's it. Nothing that I have to worry about.

"My thoughts is continue with Basel, Paris and London.

"[It] is true that the past few months I did not have the best of luck. I didn't have the best of luck since Wimbledon.

"I was playing a good year, very positive year. After that I was unlucky with the wrist, what's going on now. Now there remains three more tournaments for me."

Lopez's reward for the victory is a third round game against 13th seed John Isner, while Nadal will return to Europe for the final tournaments of the year.
