The 22-year-old forward was trying to get to the ball before Chernik but, slightly off-balance after a defender's push and too late to beat the keeper to it, he copped a headbutt to the chin.
Lengoualama seemed to have taken the roughest knock of the two and medical staff immediately put a neck brace on him before putting him on a stretcher.
Chernik also needed some medical attention, but after applying some ice to his head was able to shake off the damage and finish the game.
